EEA PR version 03/16

Hi, just started filling in EEA PR form which have confused me well....
Originally I`m from EEA country Lithuania, but have lived in UK since 2008
The problem i find that I`m not sure which sections should i fill in.
Has anyone lately filled this form in and could give me advice ?
