Financial requirement

Dear member,
i have confusion regarding my earning. i am working as a company director since 2014. and i did submit my self assessment for 2014-2015 and 2015-2016. but from april 2016 i am taking salary through my bank and every month my accountant send me payslip. so my question is :-

In FLR m form(for my wife) , should i declare my earning from a self assessment or salaried employment ?
