Applying ilr with my husband

Hi all

We are applying ilr on next month.my husband holding tier 1 general visa had completed 5 years in uk .i am also eligible to apply with him as I applied my tier 1 dependant visa before July 2012 and completed 2 years in uk.my query is what all documents should we submit from my side and from my husband side.? Should We have submit seperate
SET(O) application for both of us or a single form?looking forward for your replies and thanks in advance
