Form required for Tier 1 Dependent applying for ILR

Hello,
I am a Tier 1 dependent (since Aug 2011). My husband is up for ILR and he is going to apply for me (ILR) at the same time. He is using SET(O) as his form, but what form do I need to use?

I am confused about this because when we applied for Tier I Extension 2 years ago, we both submitted separate forms as part of the same application.

Any clarification or help would be greatly appreciated.

If you are applying at the same time with your husband one form is sufficient you don't need to fill any separate form it is clearly mention in Set (O) form.
