Payment made in advance for PEO appointment

I have made an appointment for application for ILR at UKBA's public enquiry office in Cardiff, and already made the full payment of £1426 online. In this case, should I leave the 4th page of the SET(O) application form blank, or still fill in details of my credit card used for the payment?
