How do I apply for a temporary Stamp 4 (Ireland)

Hi all,

My 5 year work permit is about to expire and I am wondering what the application process is for a temporary Stamp 4. Where do I send it to? I don't want to apply for a long term residency so I believe this is the correct route to take?
