Format of affidavit for surrender of Ind passport pre-OCI

Format of affidavit for surrender of Ind passport pre-OCI

hi,

I was recently naturalised as British citizen, but the IPS lost my Indian passport both current and previous.

I was hoping to apply for a surrender certificate before I apply for my OCI and the procedure states I need to submit a notarised affidavit for the same:

1) Does anyone have the format of the necessary affidavit showing loss of both current and previous passport. If you can post the text, it would be great?

2) Do the people in HCI, London have a person to notarise the affidavit? I have heard they charge £8 for the same...but is this the case for all forms of affidavit? Would they notarise the above in my case?

Hi - I am in the same situation. Did you or does anyone else have the answer to these questions.
