Indian passport for Indian born baby for UK resident father

Hi All,

My daughter was born in India and I am working in UK on Tier 2. I am planning to apply passport for her and it requires a sworn affidavit by the parent(s) resident abroad attested by the Indian Mission along with affidavit from parent residing in India as well (as per Annexure 'H'). Now the consular services in HCI London are outsourced to VFS, which service I need to select ?

My best guess is "Attestation of Copy of Indian Passport or any Other Miscellaneous Certificate (s) based on Indian Passport". Is it right ?

Another question, my wife's name is endorsed in my passport, but my name is not endorsed in her passport. Is it required to endorse my name in hers for applying passport for my daughter ?
