PIO Application form queries

Dear members/Moderator/Administrator,

I am filling PIO application form for my baby <1year, kindly help for the below queries:

1) 3-d - asking for Profession/Occupation, should I fill-up mine or leave it blank

2) 5 - Business/Office Address.......

3) 10-F - Any Visible Distinguishing Marks.....is it compulsory? Because as my baby is <1, no distinguishing marks....

4) Signature of the Applicant......both the parents has to sign?

Kindly help me....as soon I get I will submit the application

Mention "Child/Infant". Parent - Your profession.

2) 5 - Business/Office Address.......

Not applicable

3) 10-F - Any Visible Distinguishing Marks.....is it compulsory? Because as my baby is <1, no distinguishing marks....

Not necessary. But they might ask you to mention this while accepting your application, so if your child has a mole, then mention that there

4) Signature of the Applicant......both the parents has to sign?

Parents can sign for minors, you or your spouse can sign.
