Help with Tier 2 application form- Change of employer

I hope you are doing good !

I am applying for tier 2 visa - change of employment. I have few doubts as follows..

1. Which application form to use for change of employment for the Main Appplicant and The dependent?

2. how to find previous home office reference number for the main appilcant and for the dependent who are in current tier 2 general visa and dependent? I have my previous tier 2 visa applicaiton acknowledgment form UKBA - Should it have the reference number? Also for my wife where to find previous home office referene number for previous entry clearence?

3. Do anyone have any letter format from the employer for maintence of funds to send to UKBA please?
