TIER 1 Extension - Maternity Query

Hello,

Please can someone help me with the below query?
I am going for TIER 1 extension and I have doubt regarding the Maternity dates
I was on Maternity leave from Feb 3rd 2014 to 25th July and I re-joined my company on 28th July.
How should I show the dates in the application? Can I exclude the dates from Feb to end of July or does it has to be exact dates, if yes how to I calculate the pro-rata salary for the rest of the month.
For Example if I am claiming from 1st July 2013 to 31st Jan 2014 and July 28th to Nov 28th, how can I calculate the salary for the month November ?(i.e. from 1st Nov to 28th Nov)
Sorry if this questions was asked elsewhere, I searched extensively but no luck.

I have excluded the dates from Feb to June and included July in my earnings as it was not affecting my salary band.
