Director/ company loan agreement

Hello guys,
can I have a director/company loan agreement sample please. My extension is due in may 2016, 50k route. Best of luck to all of us.
also would appreciate if anyone could share the home office number please.need to ask them about my business partner position. He has switched from the group to ILR and i'm stuck in the middle not sure what to do. We have invested nearly all the 50kz together in our business and now he is no more on the same visa as mine. I'm not sure what to say in extention as he is no more on interprenuer route and the 50kz we spent was a Team expenditure.
