NHS Surcharge

I know new application made for a spouse visa are to pay the £600 charge.

But what if a spouse has already come over to the UK from Pakistan originally, in say March 2015. Are they now exempt from from the charge because they applied before April 6 2015 & have been successful in their visa application?

They won't need to pay the surcharge until their next visa extension.

I was thinking it might just be new applicants who will have 2 pay & then pay again at the extension.

I believe you'll find that the extension application won't go through without an IHS number showing the surcharge has been paid.

My spouse will have to pay this surcharge now at the time of his application.

So when the time comes for his extension he will have to pay another surcharge again?

That's correct. £500 the next time, not £600.
