PBS Dependent applications questions

Friends,

My wife has an appointment for the PBS dependent visa later this month. While filling the form (Tier 1 or 5 PBS dependent) we came across some questions we need help with.
1) We have paid the full fee for the PSC appointment plus health surcharge (GBP 2736) at the time of booking the appointment online, what should we fill in sections A2.7 - A2.14, which ask for the payment details? I am attaching the payment conformation email, so will it be OK if I leave these sections (A2.7 - A2.14) blank?

2) On the same page
Section 2.2: Contact name in UK if different to that of dependent? Should I put my wife's name here?
Section 2.3: Your full name: Should I put my wife's name here?
Section 2.4: Your date of birth: I guess this will be her DOB
Section 2.5: Main applicant's full name: Do we enter my name here? Or is my wife the main applicant here for the purpose of this form?
Section 2.6: Main Applicant's DOB: Same question as 2.5.
