Visa for Tier 1 ENterprenure Baby Visa

Hi

Please help me how I can apply for the tier 1 enterprenure baby visa. I have got my visa in June 2014 and my baby is 23 months. I have struggle to get passport from the indian ambassy as they took to long. and now I have maintain the balance for him now how I can apply for the dependent for him. Please help me soon as possible.

Is Child has to pay health surcharge?
Because when I am doing the application its asking me to pay.

I am doing application for Tier 1 Enterprenure child dependent. While I am doing application asking me to pay for the child health surcharge .
Is Child has to pay health surcharge?
Because when I am doing the application its asking me to pay.
