Difference in forename in passport and degree certificate

Difference in forename in passport and degree certificate

I want to apply for Tier1 Visa (General Category). I have completed all my forms and other documents and i lately realized that there is difference in the forename of passport and Degree Certificate.

Forename in Passport : SOHAM HARNISH
Forename in Degree Certificate : SOHAM HARNISHKUMAR

As per the earlier post which i have seen, it was mentioned to create an Affidavit regarding this. I have got 2 queries :

a. Do i require to create an affidavit for my case ?
b. Affidavit Creation Process. Affidavit provided by any solicitor will do or it has to be done in front of Notary.

Please help me with this query. It will be very kind of you, if you can provide me guidance.
