I have some questions on how to fill out my EU1 form...

Hi everyone,

I am American, my huband is Swedish. Now, just a coupla question before I send it out...

on the checklist it says..

1. Passport(s) or National ID (if applicable)
Q. I know my passport for sure, but do they need my husbands too?

2. Evidence of residence
Q. the Guarda wouldnt let me register my residency until I got this EU 1 back..Do they just want some kinda bill or rent agreement with my name and address on it?

3. Birth Certificate (if applicable)
Q. Who would not be applicable?

4. Medical Evidence (if applicable)
Q. What does this mean? My medical insurance from the US?

5. Other evidence of relationship
Q. isn't a marriage license enough? What else do I need?

Thank you in advance, I really want to get this thing out so I can get it back!!

hi melo,,,
it is really clear written on last page of application what u need..

1.u need to attach both original passports
2.u need any kind of proof of address on name of both of you,should be authentic like any bill or rent agremment.
3.u don't need to provide birth certs.
4.dont need any medical evidance.
5.need your original marrige cert.
6.2 photo's of both of you.

Hi Melo,
I can understand your confusion about the request on the EU1 form about evidence of relationship. I think it would be helpful to not only provide the marriage certificate, but also other documents such as bills you might have paid together over a period of time, or flight boarding passes to visit each other,airport immigration stamps, (emails also, but they are not usually taken seriously) they want to know that you have been seeing each other in person over a period of time and not had a relationship of just instant messages or text messages.Phone bills of calls made to each other would also help in a way.
