P60's are not a requirement for applying ILR although the case worker has every right to request evidence of your earnings. Did you have any absences outside UK with the current employer? If so you need to tick the box else you don't need to tick.I assume you have issues only with the previous employer for providing the letter and not your current one.

12 month maternity leave is not a problem but the important point is that in case you are travelling to your home country you should not be away for more than 180 days in any qualifying year

Usually the premium services appointment open 28 days in advance. You don't need to worry about availability as there are plenty of time slots. They also have out of hours service for a minimal fee(£ 63).
Check exactly 28 days before the intended date.

Not required. You need to fill only if your most recent payslip will have the entry for paternity pay.
