I have somehow for the life of me, misplaced the original application in hardcopy - so the only name that I could track down from my correspondences with the Home Office customer service is that it's a settlement application. I applied under https://visas-immigration.service.gov.uk/country-selection and had to pay a lot of € for it. This, given that the Home office website stated that it's free somewhere I think, but halfway through the application, one has to pay for both the application processing and the NHS surcharge.
