NCS were very particular about me being in the UK exactly 5 years ago from the date application reaches HO. May be 4 july is safe. Others/gurus might have an opinion. Good luck

I don't think it is required for employed(I didn't submit any and was approved). For self employed it is a nice to have. For EEA treaty rights route, I think it is mandatory. They have your NI number and tax reference details anyway and they can do all the checks they need - section 3 item 3.5 form ...

Worth trying the test centre where you got the pass. Guess they will help you if you explain the situation, they should have a record of all certificates they have issued.. ask them for a duplicate and you may get one if lucky!
