FLR for spouse after my ILR

Hello Forum,

I have been granted ILR few months back . My wife is still with Tier 2 dependent visa (PBS dependent). We got married before I have received ILR and she came to UK by Tier 2 dependent route.

Could you please suggest which form shall I use for her FLR application - FLR(O) or FLR (M) ?

What is the valid from date of her visa?

Did you receive ILR based on 5 or 10 years residence?
Char (CR001 not Casa)

Thanks for your reply

Her visa is valid from 19/05/2013 to 08/05/2014 - issued in UK.

I received ILR through 5 years residence route from Tier2 to ILR path.
