For Extension- Business account

Dear all ,
Could you please guide me with regards to the following for extension purpose:
1. Do you have to submit the bank statement for all the three years or only those showing the investment transaction.
2. Is it better to show the investment in terms of directors loan or share capital.
3. If directors loan, can anyone who has applied provide me the sample of the agreement letter.
4. If share capital then what needs to be done for that.

I would be really grateful if anyone can help me regarding the same.
