FPS Format

Hi Dear Members, I came across different formats of FPS on this forum and am confused now

(1) which format HO looking for ?

(2) What info HO is expecting to be included on FPS? the ones I got has got
company name,tax ref,month number and year,name,NI number,starter/Leaver, Liability
it does not have total/net pay info, will this be acceptable?
