Help in filling NHs surcharge for dependant

Hi all ,
Hope u all doing great . I am in tier 1 entrepreneur .I need ur help in filling Nhs surcharge details for my son and wife application . Contacted some visa experts through phone no one can Solve the issue .

I have filled the application . Even I took appointment . But when I fill the Nhs surcharge section , first part I provided my son's details in Nhs surcharge section . It is ok . But they are asking about main applicant details that is my details .
My visa route question : it is tier 1 entrepreneur .
But the next question is my visa type : there are five options to select , but nothing is related to my visa type :
My visa type options coming up there are ,

Tier 1 entrepreneur child
Tier 1 entrepreneur migrant ( extension )
Tier 1 entrepreneur migrant (extension ) CESC
TIER 1 entrepreneur migrant ( initial application)
Tier 1 entrepreneur migrant ( initial application ) CESC
TIER 1 entrepreneur partner .

But , My visa type is not coming under any of these I believe . Which one I select ? So confused . My appointment is on 4th August . Please help me to solve his friends .

What is the problem with "Tier1 Entrepreneur extension"?

Tier 1 entrepreneur extension is not there . It is tier 1 entrepreneur migrant extension . Do not know what it is !
