TIER 1(entrepreneur visa)+ dependent extension

Hi Guys
Can i get some details as below for the TEIR 1 (ENTREPRENEUR VISA) Applying together with Dependent

1)Apart from BUSINESS INVESTMENT (I.E £50,000) what amount should we have to maintain in our personal
account and how many months?
2)Does this amount cost of living one has to be only in my account or both accounts ?
3) Can any one send me applcation total fee & application form?
