Payment details when applying for same day app07ointment

Hi

Do I need to fill the boxes A1 and A6 to A18 if I am applying for same day appointment? The payment would already have been done online.
Also in A1 to options are £1607 (Standard) and £1552the. However when applyiing for same day appointment the fees is £2007 so I am not sure what is the best option to choose.

Can someone please help with both the queries please

leave it blank and take a print out of payment receipt sent to your email.
