Make an excel sheet in following format and get it printed on a companey headed paper. Make sure you make an arrangement with someone in office to verify this letter when you do attend PEo (if u are)

Date in Payslip Gross Income Net income

I know about Date/Month, Gross and Net columns.
I was looking for contents and to whom it addressed to? I am looking for Format including text content explaining apart from above column data(s).

Our Clients has requested his schedule of earning from ... to..... .
we are enclosing this schedule and confirm the earnings.

please do not hesitate to contacts us.
payroll

make something like this or ask payroll they normally do it themselves...

just call payroll half n hoir before you go to peo appointment and tell them they can expect a call.. although its highly unlikely but just in case let them know... so tht they can verify

In the same letter you can include..


mr xxx has not had any unpaid holiday ( will help as absence letter as well?)

I will go for Tier-extn application, so this added clause won't help here.
Even in case of ILR, that's seperate section in ILR form e.g. in SET(O).
