Tier 1 general and Dependant Application forms

I am going for my visa extension next week and would like to get confirmation on the Tier1 General and Tier1 General Dependant applications. Are these forms in the following link are valid ones?

Hi ,

I just noticed that the form which I downloaded and trying to of submit is Version 04/14 ( applications made on or after the 6 April 2014.) but I noticed there is latest version uploaded dated: 12/14 ( applications made on or after the 1 December 2014).

Can you advise me which form should I fill , going for extension on 19th of December?

Also, are there any changes for Tier1 General Migrant Dependant Application forms?

Hi and whom ever has helped;

My Application for Leave to Remain under Tier1 General PBS Category has been approved; Applied in Person on 19th of Dec, awaiting for the Biometric Card to be received.

Not sure how many year will I given? Got Visa for 2+3 and now this is 2nd renewal...
