Wondering which form I should fill out.

I am a Canadian citizen, moving to the US to study at a school. It's not a college or university, but a school which trains in the entertainment industry (comedy school -- Second City). I'd like to work while I'm there. I'm just not sure which form I should fill out, and if this school counts for anything, or if me being an "artist" counts for anything, or if me being Canadian counts for anything. If you can help, thanks.
