Change of name due to marriage

Hi,
I'm applying for Tier 1 (General) extn. I need to submit the marriage certificate as I'm applying for my wife as well. The surname of my wife differs in the marriage certificate from that on her passport. The marriage certificate shows her previous surname where as the passport has the new surname after marriage.
Do I need to include this info while answering the below question in the PBS Dependant Application Form?
"B5. If the dependant is known, or has ever been known, by any other name(s) than those given above, please give details below and send the evidence showing the name change:
Name known by Dates from and to Evidence sent
"
If so, then what could be the Dates & evidence?

No you are fine by submitting the Marriage Certificate only. Case Workers are quite aware of these. For my depedent,I submitted the passport(with new surname) + marriage certificate(with old surname) and it was accepted.

You can provide some document to link two different surnames.. like satuatory declaration saying that the name has been changed... that would give an additional info...
