ILR holder which form need to use for 03 pbs tier1 dependent

Hi Amber, Obie and other seniors

I and wife got ilr last week at peo as tier1 general 5 year route.now I want to apply ilr for me three kids (non uk born).they all have pbs dependent visas so which form I should use ....is separate form set o for each kid or one set o form neet to be use.
