Manci thank you very much, we have received today the allocation and applied two weeks ago! The only information needed is the number of allocations requested and the reason - in your case "extension of leave of sponsored migrant Mr X"
What took 3.5 months to process was probably the sponsor licence application which is not the same.

Manci, can you please advise where does Level 1 user fill in the above details? As i was told on the page "Request additional CoS or CAS" there is no option to add all the details? Be mindful, that the new start date on your new cos needs to be next date of expiry of your current cos (this is not the same as expiry of your current leave as leave is generally given 15 days more than expiry of cos).
Also he can only assign the start date of max 3 months in future and you will have to apply within 3 months of issue of cos

Also can you please advise about the CoS expiry date? We dont know what what the current CoS expiry?
your current CoS has been used and is not relevant
My current visa is until April 2015, so should i ask for CoS start day in January 2015.
what is the work end date in your current CoS?

I will, if i may ask the last question, do you know where i can find the latest updated list of all documents required to apply for the Tier 2 extension? I don't want to miss something out and apparently there are many versions to the guidance. As soon as i get the new CoS number i'm applying for an extension. At the moment from my research i've made, I have to prepare:
1. Online application from https://www.gov.uk/government/publicati ... orm-tier-2
2. Original passports + passport sized photos
3. Employer Unrestricted CoS number
4. Payslips of my salary for the last 6 months
5. Bank statements showing at least £1K in savings continuously over the last 90 days
6. Original University of Manchester Certificate
7. Payment for application
8. Post documents secured and signed for, tracked postage with a pre-paid envelope for return
4 and 5 not required for extension applications, 6 not required if you met the English requirement for your present visa.
Ask for a printout of the CoS and check if all the data in it is correct

the health surcharge is £200 p.a. but it only starts from 6 April 2015:

the sponsor may have received a general email about the health surcharge and if you applied before 6 April you shouldn't be required to pay it. Ask the sponsor about the contents of the email they received
