Bank Statement proof for past earnings

Bank Statement proof for past earnings

ICICI bank has provided me a bank statement but not on its letter head.But they have sealed and signed the bank statments for every page.Can anybody let me know whether this is fine or the statement needs to be in their letter head.

Mr.Kabaleewaran,
It is mandatory to provide the Bank staments either on a letter Head paper or else you can also take printouts given by the bank officials which is DULY STAMPED ...otherwise homeoffice may not accept your application........

Under the paymentdetails section in the HSMP application, there is a signature column. Would that be applicant's signature or the card holders signature?

Also can we use ICICI debit card for this purpose?
