Question for Tier 1 extension form .. please help

Question for Tier 1 extension form .. please help

I have 2 Work permit ( 1 from India and 1 from within UK) and one tier 1 stamped on my passport

I applied for HSMP letter in feb 2008 and the based on that i got Tier 1 General stamped ( bcos of transition arrangement as HSMP thing was removed in between).

I am applying for an extension of Tier 1 general now and have few queries ( not able to understand some points in extension form):

Tier 1 General migrant Extension form
Question 1:
B11. Home Office reference number(s) (if known):
I have got a reference number when i got my 2nd WP stamped it starts with LTxxxx (15 digit long), the space provided in form is for 13 digit. Is this the correct ref number i am looking at ?? If yes then how do i put in extra 2 digit in the form ???

Question 2:
B14. Points Based System migrant reference number (if known):
I have got a reference number when i got my Tier 1 stamped it starts with PBxxxx (15 digit long), the space provided in form is for 13 digit. Is this the correct ref number i am looking at ?? If yes then how do i put in extra 2 digit in the form ???

Tier 1 general dependant extension form
J6. Has the main applicant’s application been assessed in accordance with paragraphs 39-42 of the HSMP Forum Ltd. Judicial Review Policy Document?
What to do with this point ????

I will really appreciate if anybody can help me with these questions.
