Actually the new Aug 2016 application form states 1) 2) The ORIGINAL of your current passport and any previous passports valid during periods of residence in the State together with a photocopy of the biometric page of each passport.J_Karim wrote:You are welcome..

If you live around dublin live you can visit them or they have few branch all over ireland..

In your case you were on holiday so its not appropriate for you.
just mention back 9 years address from Ireland unless you resided some other countries
