Tier 1 General to ILR for Main and Depandent

Hello People,

I am on Tier 1 General visa (approved in March 2010 but traveled in UK for first time on 25th September 2010).
My wife is on Tier 1 General Dependent visa (entered UK on 12 March 2012).

As I am completing my 5 years today, I am now eligible to apply for an ILR. Also my wife entered UK before July 2012 as Tier 1 dependent, so she is also eligible to apply for an ILR (not ILR Dependent).

I have a query regarding the application form. I have filled in Set(O) form for myself in which I have filled the section for dependent.
But do I need a separate form for my wife? If yes, which form should I fill in for her?

Apply to settle in the UK wrote:Family members (dependants)

So I assume, I don't need to fill separate form for my wife.
