Tier 1 extension - appointment at PEO - case ID

hi folks,
I will apply for an extension in December and because I would like to make a personal application, I used the following link

http://www.ukba.homeoffice.gov.uk/book-appointment

I was asked "the case ID" in the 3rd screen. According to the explanation given in the page
Your case ID is a unique numeric reference for your application. You can find it on your invitation letter.

Can anyone clarify what this "invitation letter" is for extension of Tier 1?
I have received the HSMP back in 2008 and then delivered it to switch to Tier 1 in 2009. I do not know any invitation letter other than that? What am I missing?

These are the selections until I am asked the case ID

screen1
Which service are you applying for? => TIER1 GENERAL
screen2
Are you applying using the premium service? => NO
How many dependants? => NONE
screen 3
available from, available until, days of the week, available time etc => RELEVANT DETAILS
Select up to three convenient locations => Croydon
(after selection for the preferred time slot and place)
screen 4
click "book appointment"
screen 6

these are fields that needs to be filled before I can complete booking
Case ID*
First name*
Last name*
Date of birth* enter year in 4 digit format
UK telephone number*

maybe a related question is
Should I have submitted an application online for TIER1 Extension to be able to book an appointment?
This does not make sense though

I thought premium service is only for the same day and if I want appointment for -say- next month, I could continue without it.

unfortunately there are no available slots for premium service. It must be very limited.

Please could someone reply to this question as I am facing the same problem.

My case:
- Prefer applying in-person
- If I choose Premium service icon it says no date available for next 2 months
- But if I choose Premium service = NO, then dates are available but it asks for CASE ID which i dont have
- They are mentioning that BIOMETRIC Details can be provided on the same day when you apply in person, i guess case ID is something related to biometric forms
- I dont know why it asks for case ID, can someone explain how can i get the case ID
- I want to apply in person,

if this is a TIER 1 extension application, you must select YES to premium service (which means applying in person)
unfortunately the quota is very limited and you need to try hard to find a slot for your application.

premium service is the same day/in person service. It does not mean getting an appointment on the day you book it, it means booking an appointment in order to apply in person and (in most cases) receiving a same day decision.
