For a permanent residence card for a family member who has lived in the State for five years apply using the application form EU 3 (pdf).

Completed application forms should be returned to the EU Treaty Rights Section of the Irish Naturalisation and Immigration Service (INIS). There is a list of the documents required and frequently asked questions about the EU Treaty Rights on the INIS website.
You don't apply for an extension of Stamp 4 EUFam. You will have acquired Permanent Residence once 5 years have elapsed. It is unclear what "stamp" the DoJ are authorising / the GNIB are giving, but my guess is that it will be Stamp 5 (or Stamp 5 EUFam).

so its mean we have to wait again for 6 month to get new stamp ? because the documents would be sent over to EU treaty section.....
