About changing from Tier2 to Tier1 visa- urgent

HI,
I am here on UK with Tier2 (intra company transfer) dependant visa. Now if i would like to work here. Heard that I cannot work with Tier2 dependant visa. And needs to apply for Tier1 to apply. If so, Is it possible to apply for Tier1 visa while staying in Tier 2 ? Could some one help me.. Its pretty urgent..

1. Are you a doctor / dentist?
2. There are 5 sub-categories of Tier 1. Which are you referring to? If Tier 1 (General), the category is closed for new applicants.
