I had a similar question earlier. I travel frequently for my job, and often the passport is not stamped when I leave, only when I return. As a result, it will be difficult to ascertain how long I'm gone.
In your case, I would just be honest with the dates, and explain the absence on your passport and your use of the interim travel document.
I'm not sure of the complete answer to your question (ie; how do they track the absences). I know in the US they use flight passenger lists now, along with entry/exit tracking for foreign nationals. That could be true in the UK also, although I've never seen them scan in a passport, etc., as they do in the US when I return there.
In terms of what you should do, I would explain the differences between what you've documented and your passport. Even if they have other means of tracking the information, they could still pick up on the fact that you have listed absences that are not in line with what is documented in your passport. It's better just to explain it.
