accountant letter for dividend income from own ltd company.

hi, To claim points for my own ltd comapany dividend income(no salary ), Accountant gave me the following letter..

We are acting as accountants for abc company. We can confirm that mr xyz is sole director and share holder of abc company. As a director Mr xyz is responsible for managing day -to- day activities and is the sole beneficiary of any profits in the company and derive dividend income to extract the retained profit..
We confirm that mr xyz has received the following..
net gross.. tax

question1: is it necessary to mention that i am an employee as well?
question2: is the wording of letter o.k to claim points for dividend income of own ltd company?
please advise me..

Does letter contain details of months? E.g. in which month you received dividends? It is vital to mention the dividend period. It is good to have dividends present as they were given. If you took dividends every month or once in three month than it is good to mention the month of the dividend. This will help caseworker to corroborate the same from your bank statement.

yes, the letter contains dates of dividend payments..
but i am confused with dividend period(ie) start and end date for dividend earnings...
The story is company incorporated in feb/09 by formation agent. I bought the company in july/09 and started trading..so till jul/09 formation agent is the director and share holder..
And from aug/09 started trading and i am the sole director and share holder.. accounts prepared and submitted from feb/09 to till dec/09. i took the dividends oct/09,nov/09,dec/09.

so for the above scenario what is the dividend earning start and end dates?
please advise me, thank you,
