Address question

Hi all,
I own a home and rent another which is closer to work, they are about 150 miles apart. I have rented several houses in past 5 years around work place. I spend time in both houses. Should I begin with my permanent address when I list the addresses?
Also I note that that there is no place in AN form where I could give my work address for correspondence, as it was in HSMP,Tier1 or ILR forms.Any ideas on how to do that?
