Advice on PR appplication after ROR

May I say a BIG thank you to all moderators and dedicated contributors on this wonderful forum.

A short re-cap of my journey:
Got married: August 1st, 2009
Divorced completed: Jan 2013
Got RC (ROR) : June 2013

In 2 day’s time, 01/08/2014, it is going to be exactly 5 years since we got married.

My questions:
1. Am I right to take it that I can apply for PR any time from now since it is 5 years since the wedding?
2. What documents should I need for a successful application considering I was working all during the last five years and can provide letter of employment and P60 for the period?
3. Do I still have to include the old documents I had regarding my ex-wife’s employment etc
4. Is EEA4 the right form?
5. Lastly, any advice which might be helpful?

EEA4 would be the correct form.
