Advice for Tier-2 visa application

Hi,

I am currently on Tier-2 and my wife and one year old child is my dependant. Now I am going to change my employer as Tier-2. My query is do I need fill out separate Tier-2 Dependant application form-one for my wife and one for my child -along with my main application? Second thing it seems there is slightly different postal address for Tier 2 main applicant and Tier-2 dependant. Should I send my application to one address and my wife and child application to dependant application section?

the postal address is the same for the main and the dependant application:
P O Box 506, Durham DH99 1WB except one is marked Tier 2 and the other Tier 2 Dependant. Send two separate envelopes
