EC application in Belgium with my wife

EC application in Belgium with my wife

Hi,

last month, i received my approval and now plan to apply for EC. I have already booked my application on http://www.visa4uk.fco.gov.uk/ and will take filled in VAF2 form with myself to the Belgian embassy.

but i dont know what is required for my wife? Her application is included in my application or i have to lodge a seperate application for her?
if so , which form is used for that kind of application?
