Applying for UK tier 1 PBS Appendix 1 (Apr 2010) from India

When applying for UK Visa under Tier 1 (General) migrant from India. Have the following queries:

1) Under point 1.3 occupation - If employed - does one need to indicate the Employer name

2) Under point 3.2 Method of postgraduate degree verification - I have verifcation from UK NARIC for my Bachelors Deg and not Postgraduate Deg. Is is necessary to check any of the boxes

3) Under point 4.6 Earnings Table - does one use the multiplier factor (uplift factor = 5.3 for India). If yes, how?

4) Under point 6.3 English language test - Have passed the IELTS test. What number should be put under "Award reference number"? there dosent seem to be any 12 digit number on the IELTS report card.

5) Isnt there any Pointwise guidance for filling PBS Appendix 1 ..like there is for PBS VAF9
