August Month Restricted CoS - Pending Status - Urgent

Hi Manci / any experts,

Please help me out.
I am holding Tier 2 dependent visa and working for NHS.
Now my employer applied for restricted CoS on 5 of August 2014 but still ukba holds my application in 'pending' status.
Please explain Why the application still sits in 'pending' status.
HR explained me, I should leave home country to get restricted CoS.
please explain the procedure to switch over from Tier 2 general dependent visa to tier 2 general(employer sponsored) visa.
