Professional referee for a child

Hello,

I have the following problem. My daughter's head teacher agreed to provide a reference for her but she can't give her dob and personal address. Would HO accept school address and phone number and if yes what do I put on the online form?

School address is fine.

The issue is so they don't want to provide their date of birth.

I tried leaving g the date of birth blank but it doesn't let me so I don't know what to do.
