No case worker assigned after 4 weeks. Help urgent treatment

Hi all, I am new to this group and looking for some information about urgent treatment form. Is there any fee associated with urgent treatment form?Secondlly do we have to provide them with the proof that why we are seeking urgent treatment?

My time line is :
Application submitted :04/08/06
Ref # recd: 10/08/06 Ref#.081xxx
sent an email on 29th and responded application is waiting to be allocated to a caseworker??still no case worker assigned.
