change of personal details in passport

Hello,

I am an Indian national living in UK with Visa type as ILR.

I have recently carried out a change of name deed to add my family name as my surname and will be visiting the HCI, London within next 2 weeks to get my name changed in the passport. I understand that I'll be required to submit original copies of newspaper in India and London with the respective ad of my name change along with the required affidavit from forms section of HCI, London (form A-IX.doc).

I have some queries regarding this matter:
My Indian address has been changed sometime ago and as my passport was issued 9 years ago, it reflects my previous address only. So, can I apply for name change and address change in the passport at the same point of time? If, yes, what's the additional proof of new address will be required? Can I provide Aadhar card / Voter ID card? Original will be mandatory or an attested photo copy can do?
